Insurance & Billing
We participate with most local and many national insurance plans. However it is your responsibility to understand whether your insurance has limits on the doctors you can see, or the services you can receive.
We understand that an unexpected diagnosis can cause a strain on your finances. We accept payment by cash, check, or credit card. Our Finance office will work with you on developing a mutually agreeable payment plan for large balances. They can also assist with finding alternate funding sources for certain services.
Copayments, Coinsurance, and Deductibles
All co-payments are due at the time of service. A service fee of $10.00 will be charged for each co-payment not paid at the time of service. We will bill you any deductibles and/or co-insurance amounts due after your insurance makes a payment. Please pay this balance promptly.
If your insurance plan requires a referral from your primary care physician, you are responsible for obtaining a valid referral and presenting it to our office at the time of service. If you do not have a valid referral, you will be required to sign a waiver of your insurance benefits and pay for the visit in full.
If you do not have insurance, or we do not participate with your insurance plan, you are considered a self-pay patient. Our Finance staff will discuss fees with you prior to your appointment. We will give you a statement of any services rendered so that you may submit it to your insurance carrier.

- Martin's Point/US Family Healthcare
- Medicaid NY
- United Healthcare (all plans)
- UHC Empire Plan
- Wellcare (all plans)
